North York Adult Day Programs

Adults affected by aphasia, who need physical, social, or emotional supports

Freeman Family Day Centre: Serves older adults with physical frailty, as well as seniors with mild cognitive impairment

Samuel Lunenfeld Mountainview Club: Serves older adults with moderate to severe cognitive impairment in a secure environment

Wellness Cafe: The program serves community dwelling older adults with physical frailty as well as mild cognitive impairments

A multilingual community centre for older adults operated by Baycrest Centre

For adults with disabilities and seniors 55+ who are frail, isolated, or managing the cognitive challenges of Alzheimer’s, Dementia, Parkinson’s or Stroke.

BCCHC is a non-profit community based organization that provides health care services in a holistic manner and works with people to create safe and healthy communities. We are funded by the Central Local Health Integrated Network (Central LHIN) and the Ministry of Health and Long Term Care.

Our Day Centre is a meeting place for aging adults coping with dementias like Alzheimer’s disease

Adults 18-60 years, living with the effects of an acquired brain injury

Seniors 50 years+ that are physically frail, cognitively impaired, or isolated in the community

BOOST Adult Day Program: For individuals living with stroke, another neurological issue, or individuals who are frail.

Diner's Club Program: Older Adults to engage in social activity and gentle exercises

Transitional Adult Day Program: Physically frail seniors or those with mild cognitive impairments

A Day Away Club provides older adults with physical frailties or cognitive challenges an opportunity to socialize, be active and have fun in a safe and caring environment

Provides seniors and adults who are physically frail and/or have cognitive impairments with opportunities to socialize and participate in stimulating programs that are supportive of individuals’ needs and abilities

Individuals with developmental disabilities and dual diagnosis who are in a need of affordable and appropriate day services.
