Service Description: Adult day program * social, recreational, and therapeutic activities for post-stroke, frail, and older adults living with Alzheimer's disease in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

Hours: Monday-Friday 9:30am-3pm

 

Fees: Free to eligible Central Community Access Care Centre clients

 

Eligibility:

 

Persons who require assistance in daily living

·

Persons who require health monitoring and supervision on a regular basis

·

Persons with cognitive impairment such as Alzheimer’s disease

·

Persons who are isolated from the community due to functional limitations

·

Persons who are at risk of being left alone

·

Persons whose caregivers require regular periods of relief
