Service Description: Adult day program * social, recreational, and therapeutic activities for 12 people who may be less mobile but want to be more active and with higher care needs, such as advanced dementia, COPD and other complex, chronic conditions, in a secure group setting for part of the day * helps to support family caregivers who provide day-to-day care *

 Services offered:
• mid-day hot meal
• foot care
• assistance with toileting

 

Fees: $12 per day * subsidy available for eligible participants * additional fee for transportation
