Service Description: Adult day program * social, recreational, and therapeutic activities for people with Alzheimer's disease, related dementias, an acquired brain injury, are cognitively impaired, or are frail elderly and adults with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

• mechanical lift available

 

Fees$21 per day * subsidy available for eligible participants

 

Eligibility/ Population: People with Alzheimer’s Disease or related dementias * Community Care Access Centre care coordinators assess the needs of applicants
