Service Description:    Breathe Better is designed to help seniors in the community who are at-risk or suffering from Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF) stay healthy, active and independent * It will also improve participants’ fitness level, increase muscle strength and reduce risks of falls
